Our physicians do not require a referral. However, some insurance companies may require a referral from your primary care physician. If you’re not sure, speak with your insurance representative.
What You Need Before Seeing An Orthopedic Doctor
We want you to get the most out of your appointment with us, and taking a moment to plan will help us provide you with the highest level of customized care.
- Bring your insurance information and photo ID. (We accept most insurance plans, but if you have any questions, please contact us before your appointment or speak to your insurance representative)
- Ensure that you have your previous medical records or that your physician has forwarded those records to us.
- Bring any X-rays, MRIs or imaging scans related to your condition or injury
- Bring a list of all your current medications, and be sure to include any herbal remedies or over-the-counter medicines you’ve been taking.
- Make sure we’re aware of any drug allergies you have.
- Make a list of questions–we’re glad to take time and answer them!
- Allow time to complete new patient forms.
- If this is a worker’s compensation claim, please bring your claim number.
